💬The answer is not a different standard. It is a strengthened, better-enforced and more transparent version of these ones.💬

Scripted text menus can't co-regulate a nervous system. Humans need real facial cues and tone.

Built with clinical safety frameworks (DCB0129) + clinician escalation, but with real-time digital human video in your browser:

Keyword-matching chatbots miss genuine distress because real humans don't text in clean clinical scripts.

Nervous systems co-regulate via facial cues and vocal prosody. That's why clinical frameworks (NHS DCB0129) matter—pairing safety with real-time video:

The NHS already has DCB0129/0160 clinical safety standards - the gap isn't missing frameworks, it's that they're often treated as paperwork rather than live safety cases. Worth asking whether 'documentation' is the right lever, or whether assurance should be tied to deployment gates.

Featured on the hub, an NHS England consultation on improving their clinical risk management standards DCB0129 and DCB0160. They are seeking views on this from manufacturers and users of health and social care technology products. www.pslhub.org/learn/digita... #DigitalHealth

10/ `SAFETY.md` started back from first principles too - obtaining new copies of all the original DCB0129 and 0160 source documents, processing them into Markdown again (much faster in 2026!), and developing the 'proto-standard' to make it scale from one file up to a full CSMF + Hazard Log

8/ Earlier this year I got to know about #ArcKit, an incredible suite of LLM skills for all kinds of documentation and compliance tasks across ALL OF GOVERNMENT I.T!

github.com/tractorjuice...

I decided to contribute some #DCB0129 and #MDR documentation to see how it worked. It's amazing.

7/ Next up I worked with some friends to build a simple open source platform around this template.

github.com/turva-uk/dcb...

The idea was that the free service would allow a CSO to make and manage a few CSMFs.

No Word docs for your CSMF.
No Excel sheets for Hazard Logs.

Definitely better, but..

5/ Then I thought we could do with a GitHub Template Repo for this, so that future projects could have a very easy path to a comprehensive Clinical Safety documentation site.

github.com/turva-uk/dcb...

However the CSOs I showed this to found it was a bit too techy to get going...

4/ One of the first steps was to get a version of the DCB0129 specification that was not in a PDF.

I pulled it all out into Markdown with scripts and manual editing (in 2021 - WAY before LLMs mate)

3/ Over the years I've gathered some of the learning around 'How We Do Open Source MDR and DCB0129 Documentation' - so this thread will be a bit of a showcase of that stuff.

It's all open source.

The best ones will be the last and latest.

Please read and share with MDR / Clinical Safety people

1/ I'm now at a point where I'm super proud of the work we've done at RCPCH in our fully open #DCB0129 #ClinicalSafety and #MDR documentation over the past 7 years.

Since the very start we have always had completely OPEN Clinical Safety Management File / Hazard Log.

NHS England has written the digital spec for community pharmacy prescribing: EPS assurance, DCB0129/0160, DAPB4008, CIS2 AAL3, GP Connect.

The contractor finances all of it from £500 set-up and £525/month.
Fixed ceiling, mandatory floor.
Who funds a new entrant to build to it?

Just thinking out loud - do app developers, who's technology supports the delviery of NHS pharmacy services - complete DCB0129, and support end organisations to complete their DCB0160?
