#Deresuscitation
Furosemide (lasix) is a loop diuretic. It works via a threshold effect: to achieve diuresis, the concentration of furosemide at the site of action must EXCEED the diuretic threshold, which varies based on a number of factors.

Overloaded post-op patient in "deresuscitation phase". Boss wants to offload with albumin and furosemide together, then notices a "loop diuretic allergy", so opts for just the albumin.
This makes no sense, right?!

What I find it most useful for at the moment is in weaning patients/deresuscitation, but in the context of echo/lung ultrasound, fluid balance.

Give diuretics as boluses
look at Chloride, k+ and pH
Multimodal diuresis
Not every form of "heart failure" need diuretics in the firsts hours

I never started a "Bumex drip" (but I used "nephron bomb")
I never gave albumin to "enhance diuresis"
I never checked "serial labs"

We should not be afraid of diuresis and decongestion

Doppler tell us when to remove fluids in the ICU.
Discontinuous IRVF = 4× higher success in deresuscitation, with 87% S.
VExUS 2–3 is Sp but not S.
IRVF = a real-time marker of readiness, not long-term outcomes.

I think the last decades of Critical Care Medicine research has shifted to an emphasis on deresuscitation from an emphasis on resuscitation and to me this makes sense

(Although obviously they both are important)
