NETs (GEP-NETs) in the US. As I have said before and will continue saying, the incidence keeps rising (probably mostly from better diagnostic methods).

Cao et al. demonstrate synthetic lethality between PIKfyve and mTOR signaling in GEP-NETs. PIKfyve sustains ferritinophagy and limits lipid synthesis, whereas mTOR promotes lipogenesis and suppresses ferritinophagy. Dual inhibition disrupts lysosomal degradation, lipid metabolism, and iron metabolism, causing metabolic collapse and tumor cell death—a potential therapeutic strategy for GEP-NETs.

In summary: we should strongly consider Lutathera + SSRT over SSRT alone in our grade 2 NET patients. However, for grade 3 patients, I eagerly await the results of the Phase 3 COMPOSE trial before changing my practice entirely. ascopubs.org/doi/10.1200/...
