Online now: Inhibition of the CTP synthase 1 prevents GVHD and delays skin graft rejection revealing distinct alloreactive T cell responses
Inhibition of the CTP synthase 1 prevents GVHD and delays skin graft rejection revealing distinct alloreactive T cell responses
Allogeneic transplants can trigger graft-versus-host disease and graft rejection. Lopes Souza et al. demonstrate that deletion of Ctps1 in T cells prevents GVHD and reduces skin rejection. Furthermore, pharmacological inhibition of CTPS1 improves survival in acute GVHD, highlighting CTPS1 as a promising therapeutic target for alloreactive T cell responses.

A study in Haematologica suggests a higher-dose anti-CD25 regimen without methotrexate improves outcomes in haplo-HSCT:

• Day-100 grade 3–4 aGVHD cut to 7.9% vs 17.5%
• 2-year GRFS improved to 59.1% vs 50.3%
• Similar OS, relapse, and infection rates

A harmonised protocol for generating human extracorporeal photopheresates for research
Extracorporeal photopheresis (ECP) is an immunosuppressive therapy with long-established indications for treating T cell-mediated diseases, including steroid-refractory, acute or chronic graft-versus-host disease (GVHD), cutaneous T-cell lymphoma (CTCL) and chronic rejection after heart or lung transplantation1,2. The ECP procedure involves collecting autologous peripheral blood leucocytes by leukapheresis, which are separated from erythrocytes and platelets, then returned to patients. Passage of leucocytes through the extracorporeal circuit induces cellular activation, release of soluble mediators and metabolic products, modification of plasma proteins and fragmentation of physically damaged cells3,4. The resulting leucocyte concentrates are then incubated with a DNA-intercalating agent, 8-methoxypsoralen (8-MOP), before exposure to ultraviolet A (UVA) irradiation. In the THERAKOS™ CELLEX™ Photopheresis System, these steps are performed within the integrated, online system as part of a single clinical procedure, whereas in offline ECP systems, the collected leucocytes are transferred to separate devices for UVA irradiation5. Excitation of 8-MOP bound to DNA with UVA causes double-stranded breaks, which initiate apoptosis6. Thus, ECP results in a heterogeneous cell suspension in autologous plasma, which consists of mixed leucocyte types at various stages of apoptotic and non-apoptotic cell death. There is an emerging consensus that ECP acts primarily through mononuclear phagocytes that are driven to a tolerogenic...
