He’s not. He may have mild vascular neurocognitive disorder, but not dementia. The only caveat I have there is it’s hard to know what level of instrumental activities of daily living (IADLs) he ever had to depend upon due to wealth & attendants. Take the $$ away & what’s he truly capable of?

actually I've gotten so much worse in the past 2 months that I learned what IADLs are and how many of them I have extreme difficulty with (too many) just starting in the past couple months

thankfully this seems to have given me some leverage to move up my neurology appointment

I know I talk about this constantly it's because my health has declined dramatically and over the space of 7 months I have gone from full independence to needing in-home support to manage IADLs and yet I still have doctors trying to convince me that it's not real

#Tip: Most people need help with everything when they first come home after the hospital.

But after a few weeks or months of occupational therapy, many of us find that we can manage our ADLs better. Getting out of bed, to the bathroom, etc.

It's the IADLs that most of us need ongoing help with.

#AutChat A2 - I hide when showing my (often lack) of functioning would have huge negative consequences. Job interviews, for instance. I hid the difficulties I have with IADLs from my gender surgeon, because I wasn't going to give him a reason to deny me.

Most therapy plans start with ADLs (basic life skills) and then progress to iADLs (more complex tasks).

It's hard work, but it better prepares your loved one to
get back to their lives to the best of their new abilities.

#Tip: Be their biggest cheerleader during recovery. They really need it.

here's a sample of the message she sent me. The whole thing is this condescending but I didn't tell her that I get tired if I exercise I am always tired I told her that I am always tired that I am too tired to do IADLs. I told her that exercise causes muscle exhaustion and failure

#Fact: Even if your loved one can feed himself, he still needs a caregiver when:

He can't stand long enough to cook or wash the dishes.

He can't drive to the store to get groceries.

He has a hard time ordering food & supplies online.

IADLs are vital to live on your own independently!

#Tip: Instrumental Activities of Daily Living (IADLs) are also crucial to remain independent at home.

Cooking & meal prepping
Grocery shopping
Paying bills
Light housekeeping
Driving a car
Laundry
Medication management

These skills are just as important as ADLs for a high quality of life.

Yes. Only the wealthy will be able to pay for all of the expenses needed to maintain ADLs and iADLs. With no federal or state support those who do not have that wealth or robust social support will languish and die

I know this isn’t the focus of your post, but this is also the situation for so many elderly. They make too much for any assistance, but make too little to pay for the services they need. And so then, the family has to carry that burden, unpaid. Moving them in, assisting in ADLs and iADLs for free.
