And also that bizarre paradox that the biologic drugs used to treat autoimmune diseases like TNFa blockers, IL1b, IL17, IL23 blockers.... have also all been correlated to some extent with new onset autoimmune disease. Is this through increased infections?
1/ A long standing paradox in my mind (from always seeing immune "strength" or reactivity drawn as a spectrum from immunodeficiency to autoimmunity) was resolved this year at #AAI #Immunology2025. WHY do SO many people have both simultaneously? Each infection brings risk of developing autoimmunity!!

Fenestrated endothelium enables the extravasation of immune complexes into the synovium. There the interaction of macrophages (producing IL1b, CXCL1) and nociceptive neurons (producing CGRP) induce pain and inflammation.

Oxidized mitochondrial DNA induces NLRP3 activation in pDCs. This leads to enhanced pDC function beyond IFNa production so that they produce IL1b, IL6 and IL21, inducing Tfh differentiation and autoimmunity (in 🐭).

Post-lockdown showed more effect: lower levels of some proteins. In vitro challenge showed limited validated differences in chemokine and cytokine levels. 5 in the lockdown group upon LPS ( ↑IL1b, ↓IL8, ↓CCL2), IMQ (↑IL1b) or HIV envelop (↑IL-6) challenge.

5 in the vaccine group upon S. Pneumoniae (↓TNF, ↓IL1b, ↑CCL2), CMV (↓IL6), HIV env (↓TNF). correcting for time, the effects seemed mostly gone. This means the effects are temporary or may be noise due to high variability. A 3rd cohort, HIV-, was not the same (see alt).
