7/ The answer lies in neuronal chloride homeostasis. Neurons near HGG cells have elevated intracellular Cl-, partly due to reduced KCC2 expression, making GABAA receptor signaling depolarizing. Indeed, GAA increased neuronal firing in HGG-infiltrated brain, but not in the contralateral hemisphere.

The magnitude of the change in KCC2 protein is massive. We used a very conservative measure of membrane localization and did not quantify overall protein but judge for yourself in the images we provide. The data strongly supports KCC2 positive modulation for pain.

Overall, very exciting times ahead for KCC2 as a therapeutic target. To my knowledge, this is the first, or one of the first, validations of a pain target in human spinal cord. Thanks to the organ donors and their families for their gift that makes this exciting work possible!

🚨 New preprint from the lab!

🧠 Despite representing <5% of total #KCC2 in mature neurons, we show that the KCC2a isoform acts as a powerful developmental switch: inhibiting KCC2b early on, then boosting its membrane expression and fonction in the adult brain.

Reduced KCC2 expression in the dorsal horn links to chronic pain and opioid use. KCC2 regulates chloride gradients for inhibition, and its dysfunction drives neuropathic pain. Findings support therapies targeting KCC2.

In our (Dallas) cohort we found KCC2 membrane downregulation in donors with a history of chronic pain, and in a separate cohort of individuals with a history or opioid use disorder. Very clear evidence of decreased KCC2 function in both of these disorders.
