Know your Numbers Week (8-14th September) aims to raise awareness of the importance of checking your blood pressure. This year's theme focuses on the estimated 5 million people who do not realise they may have hypertension. According to Blood Pressure UK, 1 in 3 people in the UK are now living with high blood pressure. This can increase the risk of heart attack, stroke and kidney disease. Knowing your numbers gives you the power to act – whether that is eating less salt, being more active or seeking medical advice. Since high blood pressure generally has no obvious symptoms, the more people who test themselves, the more chance they have of controlling it and avoiding premature death. Although age is one of many contributing factors, high blood pressure can affect anyone and does not discriminate. A key focus of the Direct Enhanced Services (DES) is the proactive identification and management of Cardiovascular Disease (CVD), hypertension and raised lipids. Within RAIDR's Primary Care dashboard and Clinical module, the Cardiovascular section can help ICB users manage their population more effectively, whereas PCN and GP Practice staff can utilise the Patients and Case Finder screens to target individual care. The RAIDR team have received positive feedback from GP Practices who have used the CVD case finding screen, reporting that they have successfully identified patients who have a high blood pressure reading signalling hypertension but no clinical diagnosis on their record.
