Leber hereditary optic neuropathy (LHON) has always presented a paradox. Many carriers never lose vision, and others remain asymptomatic for decades before suddenly converting. The mutation is therefore necessary but not sufficient to cause blindness. This excellent paper by Battista et al1 (p. xxx) helps explain why.
