Progression (Stage II–III)
- Myoclonus = hallmark (sudden, stereotyped jerks).
- Seizures, dementia, vision loss → cortical blindness.
- Motor decline → rigidity, spasticity, dystonia.
- Loss of speech, mutism, vegetative state.

Stimulus-sensitive myoclonus. It's also related to his elbow jerk and thigh-slapping.

Co-causal with his wet slurring speech and lower facial droop when relaxed, which is pseudobulbar.

Root cause for both: hypoperfusion in the brain from CHF.
