Behind every report is a practice that had no visibility into what was happening until it was too late.
For a medical practice, the first notification isn't to the IT team. It's to the OAIC, and then to every patient whose data was likely accessed.

Australia's breach count barely moved: 662 to 667. Breaches hitting over a million people went from 4 to 11 (OAIC, Jan–Jun 2026). APP 11.2 already says to destroy or de-identify data you no longer need. Start with old ID scans.

Australia's OAIC opened an investigation into the maker of the HeyCyan smart-glasses app after it didn't answer the regulator's inquiries. The concern: no assurance it meets APP 1.

Give every regulator inquiry an owner and a response clock.
