Pregnant people and recently pregnant people. The care available for me when I was going through a PMAD was laughably insufficient and I I was in no way prepared to re-enter the workforce when my leave was up. And I got WAY more care than most people with PMADs.
One of the reasons I’m making this very annoying point is that I think some people believe that if we codify Roe, we’ve fixed the problem.

But the US is deeply hostile to pregnant people in more ways than just abortion.

Oh god when I was having PMADs the cognitive part was the part no one had prepared me for. I didn’t know that could be part of psychosis! My brain just…like molasses. The first antipsychotic made it worse. The second one helped so much.

When I FINISHED my SECOND stint in a partial hospitalization program for PMADs I was still suicidal and hallucinating horrible things happening to my baby. I was on medication that left my motor and cognitive abilities impaired to a degree that I couldn’t do my job.

I think they have made great strides recently but historically I dunno man. The gaping maw around any aspect of women’s health that isn’t making a baby is shocking. Pain control, PMADs, just about everything isn’t studied (by design for decades)

BIPOC and low-income mothers are both more likely to experience PMADs and less likely to receive care.

The Massachusetts Mind the Gap Coalition is working hard on a key solution: the Moms Matter Act.

PMADs can be different from pregnancy to pregnancy just like preeclampsia, gestational diabetes, morning sickness, or any other condition — and it doesn’t mean you’re doing anything “wrong” with that pregnancy vs the other 💚

We think of this as a hormone sensitivity phenotype: estradiol and progesterone follow predictable trajectories across individuals, but the brain's response may differ. We hypothesize that heightened sensitivity to reproductive hormone fluctuations may confer vulnerability to PMADs.
