This. My endocrinologist is a "transition specialist" and the most knowledgeable in the area. She basically made me prove I knew more than her to get a progesterone script. Then she filled me for progestin (synthetic prog) instead of bioidentical.

Also plenty of trans 'specialists' do not in fact know any more about HRT than the average trans person; many do in fact know less; many are antagonistic to trans people leading their own regimens and rely on outdated information and scholarship; many trans people are consequently under-dosed.
It's really easy to spot once you're looking at it, but it sneaks in by avoiding a very simple truth:
Any general practitioner has the necessary competence to prescribe HRT to trans people and monitor possible side effects.
Prescribing standard medications with known side effects is their job.

this is also, by the way, the same reason estrogen is mistakenly said to cause blood clots- birth control contains both estrogen and progestin, but negative side effects of just progestin are mistakenly attributed to HRT as a whole, even though trans women do not take progestin.

This is likely due to ⬆️ sustained levels with TD administration.
Vaginal administration risk likely lies between TD and oral due to 3rd gen progestin used.
Progestin and thrombotic risk is less commonly discussed but also important.
