I do this for RCTs! Hypothetical treatment is three diff exam study guides. 😆

Wanna see RCTs....

He should be the placebo on all RCTs. Maybe his peptides will save him

Was bleibt da jetzt über?

Ja, wir brauchen RCT, um aus off-label on-label zu machen - alleine wegen der Verordnung.

Gleichzeitig gibt es, auch für simple Kliniker wie mich erkennbare, Probleme bei der Gestaltung mancher RCTs.

Ha! Though to be pedantic, you need:

1) large N
2) a very rich set of covariates
3) at least some RCTs to show that your observational version is informative

So Keith got it 1/3 right!

Not a modeller myself, but while on duty at the RCTS stall in Sophia Gardens, I made sure to take in the Cardiff Model Railway Show layouts!

I've run cluster-randomized trials using stepped-wedge and similar designs. Ethics is somewhat complex but not that difficult. Certainly there are no insurmountable barriers to running RCTs on even complex interventions, and we should strive to do more of them.

A separate question would be a mask mandate *in a particular context* (eg doctors waiting room) reducing risk of people getting infected *in that location*. Could imagine RCTs, but not sure how you'd measure the effect (unless people aren't getting infected anywhere else)

It's the perfect demonstration of how vacuous the MAHA movement is: they keep saying they won't take vaccines "until there are more placebo-controlled trials" & yet they're out there peddling entirely untested peptides that have zero safety or efficacy data from rigorous RCTs. Make it make sense!
The Venn diagram of the people who refuse COVID19 vaccines and the people who will buy untested, unapproved injectible peptides from a compounding pharmacy is a perfect circle.

There we disagree. I think Ioannidis is eliding two separate issues - whether mandates reduce the spread of infection and whether masks themselves "work" - but certainly RCTs can - and were! - used to measure mask mandate effects.

Ultra-low-dose #naloxone (50–100 ng) with local anesthetics may prolong analgesia & reduce opioid use in nerve blocks/IVRA (7 RCTs, n=564). Promising but limited evidence; more trials needed. #Anesthesia #PainManagement onlinelibrary.wiley.com/doi/10.1002/...

All those randomized trials he wants repeated though! For the things that already have RCTs

The RCTs I saw were always "giving out masks" or "asking people to wear masks (as part of a scientific study with no legal mandate)" - do you have a link to an example of an RCT for a legal mandate? (Curious about how - could they fine people for non compliance? And what was the messaging?)

The SHR-5 extract used in multiple RCTs lowered cortisol awakening response and burnout scores after 28 days of daily use (Olsson et al. 2009).

RCTs ongoing to see if they prevent and treat heart failure, which are very likely to make them the “fifth pillar” of guideline directed medical therapy.

When @jonathanhaidt.bsky.social said that phone bans had initially negative followed by positive effects people here said he was just trying to wriggle out of unfavorable RCTs.

But in fact that is exactly what we see in this large DiD study in QJE. academic.oup.com/qje/advance-...

You're right, there were no RCTs on mandates precisely (the Danish healthcare worker study was sort of a mandate but not exactly). I do think such studies could have been run, but no one ever did. Perhaps a better way to put it is that mask promotion and free masks had a modest impact on spread.

One word to consider when asking for RCTs - ethics
Asking HCWs to unmask during a pandemic is probably not the best idea ever suggested.

💊 β-Blockers in Hemodialysis: Does Dialyzability Matter?

Nondialyzable β-blockers were associated with fewer cardiovascular events, particularly early after HD initiation.

Promising findings, but RCTs are still needed.
