Ciliochoroidal Leiomyoma Mimicking Uveal Melanoma
A 42-year-old woman was referred for suspected ciliochoroidal melanoma. Slitlamp examination showed a pigmented mass touching the crystalline lens (A). Ultra-widefield imaging and B-scan ultrasonography revealed a dome-shaped ciliochoroidal mass (B, C). Although melanoma was suspected, intraoperative transillumination revealed light transmission without a distinct tumor shadow. The tumor was subsequently removed by endoresection. Histopathology revealed spindle cells with eosinophilic cytoplasm and ovoid nuclei (D).

Ultrasound Invention: How Medical Ultrasound Was Developed
Medical ultrasound developed through several independent advances in acoustics, industrial testing, tissue imaging, cardiac recording and obstetric scanning rather than from one isolated invention. Invention Name Diagnostic Medical Ultrasound (Ultrasonography) Short Definition A diagnostic method that sends high-frequency sound into the body and converts returning echoes into information about internal structures, motion or blood flow. Approximate Date or Period Experimental medical work from the 1940s; clinically practical imaging developed mainly during the 1950s–1970s.

Endogenous Klebsiella pneumoniae Endophthalmitis with a Large Subretinal Abscess
A 40-year-old woman presented with fever and painless decreased vision. Vitritis and a large, yellow, elevated subretinal lesion accompanied by Roth spots and retinal hemorrhages (A) were found. B-scan ultrasonography showed a dome-shaped, moderately reflective lesion measuring 2.24 × 8.16 mm at the superior posterior pole (B). Aqueous humor next-generation sequencing and culture were negative, whereas vitreous next-generation sequencing identified an extended-spectrum β-lactamase–producing Klebsiella pneumoniae strain.

Spontaneous Regression of Subretinal Fluid in Advanced Coats’ Disease
A 2-year-old boy presented with esotropia for 9 months. Examination was unremarkable in the right eye and revealed retrolental total exudative retinal detachment in the left eye (A and B). B-scan ultrasonography showed no calcifications or mass lesions, and magnetic resonance imaging was normal, excluding retinoblastoma. A diagnosis of Coats’ disease was made, and the patient was scheduled for external drainage of subretinal fluid after 1 month. One month later, examination revealed spontaneous resolution of most of the subretinal fluid (C and D).

Uveal Effusion in Mucopolysaccharidosis Type II
A 36-year-old man with mucopolysaccharidosis type II was referred for left-eye visual loss (light perception). Ocular surface vessels were dilated and tortuous bilaterally (A). Right optic disc edema was present (D); a bullous retrolental retinal detachment obscured the left posterior pole (B). Ultrasonography showed bilateral choroidal thickening (C). A topical prostaglandin analog was initiated. At 2 months, the left detachment had partially resolved, revealing left optic disc edema; visual acuity was counting fingers (D).

Ectopic pregnancy should rarely be lethal...Ectopic pregnancy accounts for about 4% of the roughly 20 annual pregnancy-related deaths in Canada, according to the Canadian Medical Association Journal. [1] (www.cmaj.ca/content/173/...)
Diagnosis and treatment of ectopic pregnancy
ECTOPIC PREGNANCY IS A LIFE- AND FERTILITY-threatening condition that is commonly seen in Canadian emergency departments. Increases in the availability and use of hormonal markers, coupled with advances in formal and emergency ultrasonography have changed the diagnostic approach to the patient in the emergency department with first-trimester bleeding or pain. Ultrasonography should be the initial investigation for symptomatic women in their first trimester; when the results are indeterminate, the serum β human chorionic gonadotropin (β-hCG) concentration should be measured. Serial measurement of β-hCG and progesterone concentrations may be useful when the diagnosis remains unclear. Advances in surgical and medical therapy for ectopic pregnancy have allowed the proliferation of minimally invasive or noninvasive treatment. Guidelines for laparoscopy and for methotrexate therapy are provided.
