In a recent issue of Molecular Cell, Genoyer et al. show that during West Nile virus infection, escaped, cytoplasmic, anti-genomic negative-sense viral RNA triggers an antiviral response via RIG-I activation. Flaviviruses typically hide −vRNA in replication compartments to avoid detection, but some −vRNA leaks, possibly aided by viral capsid proteins.

Persistent systemic inflammation may lead to the production of cytokines and chemokines, including IL-6, IL-8, IL-1β, TNF-α, and IP-10 [90,91], as well as the overactivation of the immune system, T cell exhaustion, and the generation of reactive oxygen species. Increased blood–brain barrier (BBB) permeability may allow cytokines to penetrate the brain and induce neuroinflammation [92,93,94,95,96,97]. A more porous BBB may also permit direct viral invasion of the brain. Tissue hypoxia may occur due to microclot formation.
