"Doubling up on Aspirin," he says. More likely, he is on a prescription antiplatelet or anticoagulant. But admitting to that what would mean admitting he has a serious medical issue.

Neither of these are doses of aspirin. Also, aspirin is an antiplatelet, not an anticoagulant (blood thinner). Also, the president is one head bonk away from a brain bleed.

Demographic diversity in platelet function and response to antiplatelet therapy
Recent studies have highlighted the complexity of platelet biology, revealing their diverse roles beyond hemostasis. Pathological platelet activation is now recognized as a key contributor to thrombosis and inflammation that are both central to cardiovascular disease (CVD). Emerging research emphasizes the significant impact of demographic factors – such as age, sex, race, and ethnicity – on CVD risk and responses to antiplatelet therapies. These population-based differences, shaped by genetic and non-genetic factors, highlight the need for reevaluation of antiplatelet strategies. We address current knowledge and emphasize the pressing need for further research into platelet biology and cardiovascular outcomes across diverse populations. In this review we advocate for tailored therapeutic approaches in CVD based on the recent demographic-focused findings.

Paper in this week's NEJM shows that in patients with atrial fibrillation and stable coronary artery disease, edoxaban monotherapy is superior to the combination of edoxaban plus an antiplatelet drug. Major difference was in bleeding rather than reduction in ischaemic events.

In low-risk patients with MI and early complete revascularization, stopping aspirin after 1 month and continuing P2Y12 monotherapy was noninferior to dual antiplatelet therapy for ischemic outcomes and led to reduced bleeding at 1 year. Full TARGET-FIRST trial results: nej.md/3UKJxS7

I want to talk about a patient who died when I was a trainee.

He presented with an MI & was treated w/ the usual therapies along with an eptifibatide infusion (an antiplatelet gtt which was commonly used back then).

He developed agitated delirium refractory to numerous medications… #EMIMCC #1/5

Does DAPT need to be held prior to LP?

While it's safest to perform an LP off DAPT/antithrombotics, if an LP is pressing and it's not feasible to wait, this cohort of 100 LPs done on DAPT had a 4% rate of "bloody" taps and 0 epidural hematomas

I took my last dose of Brilinta, a prescription antiplatelet medication used to lower the risk of heart attack, stroke, and cardiovascular death in patients with certain heart conditions.

I’ve been on it 2x a day since my NSTEMI on 12/23/24. My cardiologist is thrilled with my lab numbers.
