#tandem25 #bmtsm Perales: cGVHD incidence decreasing significantly due to advent of PTCY, TCD (in vivo and ex vivo), etc. Risk factors for cGVHD now better understood. NIH defined and atypical (some not so atypical, IMO, particularly muscle cramps) features of cGVHD shown nicely in 3rd slide.

Chronic graft-versus-host disease (cGVHD) is a major complication of allogeneic haematopoietic cell transplantation. cGVHD has a heterogeneous biology and morbid manifestations, affecting 30–70% of recipients and substantially impairing quality of life and is the leading cause of non-relapse mortality in allo-HCT recipients. This immune-mediated condition arises from complex immune dysregulation involving B cell and T cell activation, regulatory T cell dysfunction, and fibrosis driven by macrophages and fibroblasts. Glucocorticoids remain the first-line treatment; however, ~50% of patients develop steroid-refractory or steroid-dependent cGVHD, necessitating prolonged immunosuppression causing considerable toxicity. Four second-line treatments have been approved by the FDA — ibrutinib, ruxolitinib, belumosudil and axatilimab — targeting B cell signalling, JAK–STAT, ROCK2 and CSF1R pathways, respectively, and emerging therapies such as rovadicitinib show promise. #ASH24 #bmtsm Popat: 2yr relapse 16%, NRM 22%. aGVHD gr3+ 14%, mod+ cGVHD 8% at 24mos. No diff in RFS/OS for younger vs older pts. All pts had AML or hrMDS amongst whom TP53m relapse rate was 36.4% vs 10.5% in TP53wt.

The type of cGVHD 👇 reminds me why I vowed NEVER to use tac/mtx again. With ptCY & other data IMO it is no longer acceptable.

#tandem25 #bmtsm Perales: so far only Rux has been studied in Ph3 RCT for treatment of SR-cGVHD. Both Ibrutinib and Belumosudil approved on Ph2 studies without comparator. Real world data with IBR not as favorable as on study.
