CKD3 pt, Hx hypercalcemia, primary hyper para, s/p parathyroid resection, normalized Ca, PTH still slightly elevated. Lumbar T-score -2.5. No hx of fragility fx. Fracture doc wants to give denosumab. Don’t do bone biopsies at our hospital. I’m leaning no…but yes bisphosphonates. thoughts? #askrenal

CysC can be higher in inflammatory states. I have a couple pts with Cr-CysC discrepancies. If it'd add value, would do nmGFR or 24h CrCl & CUrea (averaged). If I couldn't find reason to distrust CysC, I'd just roll with CKD-EPI Cr-CysC and manage as ckd3

Prior to sglt2i being popularized I would not have done RASi for non diabetic ckd2 with a1 or a2. Heck, even ckd3 A1 May not have that much benefit from RASi.
I’m hard pressed to start sglt2i on everyone like I feel we’re being persuaded to.

Ok but I had a deal with my nephro partner down the hall to see my diabetics when they hit CKD3 just to show them what kidney disease really looks like and how they very much don’t want to go down that path…
