Research shows: Rifaximin prophylaxis drives resistance to daptomycin, an unrelated, last-resort antibiotic

Rifaximin is used for hepatic encephalopathy in liver patients at high risk for infections, including VRE treated with daptomycin. The antibiotic was thought to pose a low resistance risk

Why does daptomycin resistance appear so fast in Enterococcus? We finally have a clue.

DAP resistance in enterococci pops up quickly. What’s been missing is why resistance-associated membrane changes look the way they do, and why the classic path of mutations is so predictable.

LRS: daptomycin is one of the antibiotics used as a heavy hitter for gram-positive germs resistant to other stuff, think MRSA. It's got shitty side effects which are all better than dying.

Seeing it is a big "oh fuck nosocomial infections are here" signal.

11-year on Enterococcus in Riyadh, Saudi Arabia hospitals:
​• High-level gentamicin resistance declined, susceptibility to linezolid, daptomycin & tigecycline stayed high (>95%).
• VRE rates & MDR remained unchanged, with E. faecium linked to higher mortality.

Development of off-target antibiotic resistance is always a worry.

Rifaximin is used as prophylaxis for hepatic encephalopathy in patients with cirrhosis.

This study indicates rifaximin may select out resistance to unrelated daptomycin
