An important fact:

“Between 75% and 90% of home based care at the end of life is provided by unpaid carers, often family members.”

And when that’s 24 hours around the clock, that’s really hard, especially when it’s just one or two people trying to do it.

Many teams are using the excellent “Clinical Frailty Scale” developed by the wonderful @krockdoc.bsky.social.

(Often referred to as “the one with the pictures”!)

BUT it needs to be used properly, so I was pleased to see this infographic with some top tips on how to do that.

I’ve found the #AnSky starter pack - thank you @sethina.bsky.social!

Loving these #StarterPack things by the way 😅

Any other pointers to #MedSky #MedEd #PalliMed #EOLC ones?

Much like libertarianism in general. The EOLC bill is a nice idea IF you don’t live in a late stage capitalist society.

Another interesting @bmj.com piece for us all this morning.

Anita Hannig on the medicalisation of dying and how we would do well to reclaim it as a human event rather than a clinical one.

A lot of care happens outside hospitals and hospices…

“Seriously ill and dying people spend only about 5% of their final year of life in the direct care of healthcare services, with friends, family, and community members without healthcare training providing much of the remaining support.”

“But none of this will succeed without death and grief literacy: the knowledge, skills, and confidence to face mortality with our eyes open.”

⭐️⭐️⬇️⬇️⭐️⭐️

“But, when clinicians avoid discussing death and patients’ preferences for the end of their lives, patients and their families lose crucial opportunities to connect, prepare, and ultimately make informed decisions.”

Well said, @lucyselman.bsky.social!

But the stories at the time of people *deliberately* not helped with eating and drinking are nothing to do with good practice guidance trying to formalise/standardise elements of food EOLC - they were about bad training/communication/implementation

While we’re on the subject of End of Life Care, this is a blog I wrote a while back, on looking after people who might be dying.

It was written with junior doctors in mind, to help them navigate waters that I had found tricky.

Clinicians agree that noisy, crowded, impersonal emergency departments aren't ideal #eolc settings, but many patients with palliative care needs also need emergency care. Read about research exploring patient & family perspectives. #HAPC #EMSky #AcademicSky #EmergencyCare #TeĀrai #ProudSupervisor
