#PathSky Hand mass, 32M. Bland spindle cell tumor with collagen trapping- just an FH, right? Also some less bland epithelioid areas. This is a predominantly spindled epithelioid sarcoma- “modulation” from epithelioid to spindled cells is an under appreciated morphological hallmark. See Comment.

#PathSky 32M with extra-axial, dural-based, frontoparietal mass. Clearly malignant epithelioid cells arranged in cords and nests in a sclerotic background. Looks a lot like a sclerosing epithelioid fibrosarcoma, and it is strongly MUC4-positive. So, we’re done, right?

#PathSky 43F with a solitary lung mass. Relatively circumscribed, nests and interesting micropapillae of lightly eosinophilic epithelioid cells. Negative for everything except TFE3. Dx?

#PathSky This one definitely needs more than 4 images, so go to the Comments too. 65F, iliac crest lesion. The original pathologists quite reasonably considered an epithelioid hemangioendothelioma, although it was CAMTA1-negative.

Epithelioid angiosarcoma with extensive intravascular growth. Obviously there must be a right sided primary. There’s supposed to be a liver mass.

This is actually a rather spectacular example of dedifferentiated liposarcoma with epithelioid/ epithelial features, a rather rare but reported phenomenon. pubmed.ncbi.nlm.nih.gov/28719466/

At high power, the cells in this mass are rather bland, epithelioid, and somewhat myoid. HMB45, Melan A and cathepsin K are positive. Lipomatous angiomyolipoma. These can be sneaky….

Epithelioid GIST can be tricky, especially when metastatic, as it can present variable histomorphology (neuroendocrine-like, plasmacytoid, atypia, etc.) and aberrant staining patterns (e.g. keratin). This is a liver biopsy. #PathSky #GIpath

It’s also got these interesting vacuolated epithelioid cells, which are CD31-pos. The spindled cells express SMA. Nice example of spindle cell hemangioma, an IDH1/2 mutated benign vascular tumor strongly associated with skeletal cartilage tumors (Mafucci syndrome).

Intrauterine perivascular epithelioid tumor (PEComa) exhibiting HMB45 expression by IHC. This lesion was small and lacked atypia or mitotic activity, predicting benign behavior.

It was sent out, and another place called it an epithelioid hemangioma. Interesting idea, because it does have some capillary-type channels. But not very epithelioid, and FOSB was negative (not shown).
