On F:NV it was a lot of fun writing failure replies for threshold checks. The rules were 1) picking the failure option should be entertaining 2) picking the failure option should never put you in a worse position than you were already in.
disco elysium did a good job at this by making the results of losing the rolls funny. i never reloaded on disco. Design Failure
