[Why] is furosemide susceptible to malabsorption from "gut edema"?

When a patient with heart failure is hospitalized with congestion I often hear "let's use IV furosemide; they're probably not absorbing the PO."

It's a comment unique to furosemide.

But is it accurate?

Torsemide is proposed to have clinically important pharmacokinetic and pharmacodynamic advantages over furosemide. This study found no meaningful pharmacokinetic/pharmacodynamic advantages for torsemide versus furosemide. bit.ly/JASN0481

How I consult on cardiorenal syndrome:

• Bump furosemide from 20 to 60 mg bid IV
• Consider rapid resumption of sacubitril/valsartan and empagliflozin. Let's not treat these life saving medications as nephrotoxins.

He could also be wearing compression socks for prevention of blood clots. And he might also be taking Furosemide (Lasix - a prescription diuretic) for fluid retention.

Mostly likely furosemide, and he's very like wearing a catheter. His heart is no longer functioning properly and it's causing him to retain fluids (hence, the "cankles"). The intravenous diuretics remove excess fluid from the body.

These are not the diuretic trials we need. 2 diuretics will generate more urine then 1
