The key findings of the current analysis are that low ISM2 was the strongest first-trimester protein signal of 2,904 targets studied in relation to PE and FGR and that the protein appeared to have a critical role in determining the invasiveness of EVT. The findings are consistent with a causal role of low levels of this protein in the early pathophysiology of PE and FGR. Several features of the analysis support a causal association. First, the association between ISM2 and the clinical outcomes was the strongest of 2,904 proteins studied. It was the only protein that was associated with all four outcomes. Moreover, it was the top predictor of three of the outcomes and the second top predictor of the fourth, and the associations were virtually identical in independent validation cohorts. Second, the direction of association was consistent with the known pathophysiology: low levels of the protein would be expected to be associated with impaired trophoblast invasion, which is thought to be causally involved in the development of both PE and FGR.
