"Maximaltherapie erwünscht" setzt ein rationales Therapieziel voraus.

In der Konstellation sollte man einmal einen Schritt zurücktreten, einsehen, dass es kein reales kuratives Therapieziel mehr gibt, und eine Palliation einleiten.

Das scheint die *maximale rationale Therapie* zu sein.

Patients struggle to get adequate care. There’s no focus on palliation to reduce symptoms & improve quality of life.

We’re often subjected to a merry go round of tests & failed treatments or gaslighting & disbelief.

Our energy levels aren’t taken into consideration either /2

Uanset diskussion om aktiv dødshjælp bør det jo være ja til bedre palliation med omsorg, pleje og smertelindring ved livstruende sygdom.

Trist, men nok desværre rigtigt, at tilslutningen til aktiv dødshjælp øges, hvis vi ikke har tilstrækkelig palliation.

I think of this as the intersection where palliation can increase the possibilities of healing through the alleviation of suffering.

We can sometimes die while we work on eradicating an illness

We can sometimes focus on alleviating suffering and end up creating the conditions for cure

Both/and
In moments of uncertainty I like to think about "good bets" -- choices and actions that seem most likely to be helpful no matter what comes next. Here are a few on my mind. What would you add?
