I would honestly suggest doing a pharmacogenomic test to see which variants you have so you know which approach you need to target, but if your cardiologist insists, pitavastatin has the fewest side effects and is least likely to cause both MCAS and muscle fuckery.

BHIVA’s recommendation followed the REPRIEVE Study’s findings which suggest that statins could prevent as many as 1 in 5 major cardiovascular events or related deaths in people with HIV.

Eli lily went to court and secured the generic pitavastatin of Livalo for a longer than typical period of time keeping the drug prices in the hundreds of dollars per month.
Lack of regulation, control of generics, and greed causes these drug prices. Our country refuses to actually take charge.

Effect of Pitavastatin on Epigenetic Aging Biomarkers in People With HIV: Pilot Substudy of the REPRIEVE Trial
People with human immunodeficiency virus (HIV, PWH) exhibit increased cardiovascular disease (CVD) risk and accelerated biological aging. REPRIEVE demonstrated that pitavastatin reduced major adverse cardiovascular events (MACE) in antiretroviral therapy (ART)-treated PWH with low-to-moderate traditional cardiovascular risk. It remains unknown whether statin therapy can modulate epigenetic aging in PWH.MethodsWe assessed epigenetic aging biomarkers using DNA methylation profiles from peripheral blood mononuclear cells (PBMCs) in a subset of 99 randomly selected US REPRIEVE participants (65 pitavastatin, 34 placebo) at baseline and 24 months. The primary outcomes were changes in second- and third-generation epigenetic clocks PCGrimAge (trained on mortality risk) and DunedinPACE (trained on rate of age-related multi-organ decline).ResultsMedian chronological age was 57.0 (Q1, Q3: 56, 58) years and 100% of participants demonstrated epigenetic age acceleration, measured by the difference in PCGrimAge and chronological age (median difference 7.08 years [Q1, Q3: 4.69, 9.64]) at entry. Over 24 months, PCGrimAge remained accelerated with no significant differences between treatment arms (P = .89). However, the median pace of aging by the DunedinPACE increased in the placebo arm (0.036, Q1, Q3 [−0.018, 0.10], P = .021) but not in the pitavastatin arm (0.001, Q1, Q3 [−0.031, 0.036], [P = .77]), treatment group difference (P = .049).ConclusionsIn this pilot study of REPRIEVE, epigenetic age acceleration was demonstrated at trial entry. The biological pace of aging increased over 24 months in the placebo group as compared to the statin group. These preliminary findings suggest pitavastatin may prevent an increase in the pace of biological aging in PWH and support further research into statin therapy as a potential intervention to mitigate accelerated aging.Clinical Trials RegistrationNCT02344290 (date of initial registration: 22 January 2015).

Frailty was associated with an increased risk of MACE in the REPRIEVE study. However frailty did not seem to impact the effectiveness of pitavastatin on preventing MACE outcomes.

Caveat limited sample size and events in this relatively young cohort assessed with low-mod CVD risk. #CROI2025

Types of Myocardial Infarction in People With HIV in Switzerland
Of myocardial infarctions (MIs) recorded in 2 large human immunodeficiency virus (HIV) observational studies from North America, approximately half were classified as type 2. In the REPRIEVE clinical trial of pitavastatin versus placebo in people with HIV (PWH) (<3% of participants were from Europe), 20.6% of MIs were type 2. The proportions of type 1 MI (T1MI) and type 2 MI (T2MI) in European PWH are unknown.MethodsThe study included a retrospective record review, ascertainment of prospectively recorded and medically validated MIs, differentiation of T1MI and T2MI, and MI time trend analysis in the Swiss HIV Cohort Study (1 January 2000 to 31 May 2021). Exploratory analysis was performed of the associations of T1MI and T2MI with blood leukocyte count and 2 validated genome-wide coronary artery disease–associated polygenic risk scores (metaGRS and GPSmult).ResultsBetween 2000 and 2021, 16 027 Swiss HIV Cohort Study participants accumulated 181 598 years of follow-up, and 379 had a validated first MI. Of these participants, 359 (94.7%) had T1MI, and 20 (5.3%) had T2MI. Invasive coronary angiography was done in 95% and 60% of participants with T1MI and T2MI, respectively. We found no evidence for increasing or decreasing incidence trends over time for T1MI (P = .86) or T2MI (P = .85). Participants in the highest quintile for leukocyte count, metaGRS, and GPSmult had significantly increased adjusted odds ratios for T1MI; power was limited for detecting associations with T2MI.ConclusionsThe proportion of T2MI in PWH in Switzerland is approximately 5%, consistent with data from the general population and lower than in previous North American reports among PWH.

Prognostic Factors of Physical Function Decline Among Middle-Aged Adults With HIV
Pitavastatin to REduce Physical Function Impairment and FRailty in HIV (PREPARE) found small declines in physical function overall among people with HIV (PWH). However, there was substantial individual variability. The purpose of this prespecified exploratory analysis was to identify the PWH at greatest risk for physical function decline.MethodsParticipant-specific annualized rates of change on annually measured chair rise rate, gait speed, the modified Short Performance Physical Battery (composite of the latter 2 plus balance time), and grip strength were estimated from linear mixed-effect models. Change in performance that was below the 20th percentile of the study population in ≥1 measure was classified as physical function decline. Associations between baseline factors and physical function decline were evaluated with log-binomial regression models.ResultsOf 569 participants (81% male, 52% White), the median age (Q1–Q3) was 51 (47–55) years. Half (52%) of the participants had decline in physical function. The risk of decline was higher among females (relative risk [RR], 1.32; 95% CI, 1.12–1.55) and non-Whites (RR, 1.23; 95% CI, 1.05–1.45) and tended to increase with age (50–55 years: RR, 1.04; 95% CI, 0.86–1.26; 55+ vs 40–<50 years: RR, 1.17; 95% CI, 0.98–1.39). In models adjusted for age, sex, and race, we found greater risk of decline among those with history of depression treatment, higher body mass index (BMI), preexisting functional impairment, frailty (by index), and higher baseline high-sensitivity C-reactive protein and interleukin-6 levels.ConclusionsPWH with history of depression treatment, high BMI, or levels of inflammation and those showing early signs of functional impairment may be at higher risk of physical function decline and should be targeted for early interventions to preserve physical function with aging.

#JCLSpotlight: Over 2 years, treatment with pitavastatin calcium of CDO for adolescents resulted in no changes in vascular measures. Statin therapy safely lowered atherogenic lipid particles, potentially reducing cardiovascular risk.
