🆕IL-6 blockade for thyroid eye disease

Our Phase III SatraGO-2 trial: satralizumab hit the primary endpoint. Proptosis response 52.9% vs 23.4% at wk 24. SatraGO-1: same direction, not significant.

Better CAS & diplopia. No hearing loss. No hyperglycemia.

Innovent Announces Phase 3 RESTORE-3 Study of SYCUME® (Teprotumumab N01 Injection) Met Primary Endpoint in Inactive Thyroid Eye Disease
SYCUME® Significantly Improved Proptosis (Response Rate 60.4% / LS Mean Change from Baseline -1.88 mm) with Favorable Safety Profile in Participants with Inactive Thyroid Eye Disease(TED) SAN FRANCISCO and SUZHOU, China, Oct. 8, 2026 /PRNewswire/ -- Innovent Biologics, Inc. ("Innovent") (HKEX: 01801), a world-class biopharmaceutical company that develops, manufactures and commercializes high-quality medicines for the treatment of oncologic, autoimmune, cardiovascular and metabolic, ophthalmologic and other major diseases, announced today that its independently developed recombinant anti-insulin-like growth factor 1 receptor (IGF-1R) antibody, SYCUME® (Teprotumumab N01 Injection, R&D code: IBI311), has achieved its primary study endpoint at Week 24 in a multicenter, randomized, double-blind, placebo-controlled Phase 3 clinical study (RESTORE-3) in Chinese participants with inactive thyroid eye disease (TED). In this study, SYCUME® demonstrated remarkable efficacy in reducing proptosis in participants with inactive TED, with a favorable safety profile. SYCUME® is the first IGF-1R antibody drug in China and the second one approved worldwide, filling a 70-year treatment gap in the domestic TED field. Officially included in China's National Reimbursement Drug List on January 1, 2026, SYCUME® substantially alleviates the financial burden on patients compared to the high cost of overseas counterparts, significantly enhancing drug accessibility. Necrotizing Granulomatous Orbital Inflammation Associated with Crohn Disease
A 30-year-old woman with Crohn disease presented with progressive bilateral periocular swelling, ptosis, conjunctival injection and chemosis, severe proptosis, ophthalmoplegia, and periorbital pain (A1–A3). Magnetic resonance imaging demonstrated diffuse infiltration of the lacrimal glands, orbital fat, extraocular muscles, and periorbital soft tissues (B). After orbital biopsy (C), histopathology (D) revealed granulomatous vasculitis with palisading epithelioid histiocytes (arrows), fibrinoid necrosis (asterisk), and necrotizing and nonnecrotizing granulomas in a background of fibrosis (not shown).

Frontal Sinus Bone Wax Complicated by Cyst Masquerading as Meningioma Recurrence
A 70-year-old man presented with left periorbital pain. He had a history of frontal lobe meningioma resection 9 years prior. Examination revealed left proptosis, inferior globe displacement, and restricted upward motility. Computed tomography showed a mixed-density lesion in the superior left orbit, with a partial defect of the superior orbital wall. Magnetic resonance imaging demonstrated a cystic extraconal lesion, isointense on T1 (A), hyperintense on T2 (B), with peripheral enhancement (C) and inferior globe displacement.

Presumed Multifocal Orbital Recurrence of Pleomorphic Adenoma after Orbitotomy
An 80-year-old woman presented with 4-mm relative left-sided proptosis over several years. Approximately 30 years previously, a pleomorphic adenoma of the left lacrimal gland had been excised via lateral orbitotomy with intraoperative frozen section. Computed tomography showed multiple discrete nodules involving the left orbital roof, lateral wall, and temporal fossa (arrows) with bony remodeling and focal defects of the frontal sinus and anterior cranial fossa (A–D). The multifocal distribution was attributed to seeding secondary to violation of the original tumor’s capsule.

It's interesting that most of the things that Wessely mentions that "switch the other way" are just yet more examples of what Davey Smith mentions.

Visceral proptosis [sic] (or visceral ptosis or visceroptosis) is commonly seen in Ehlers-Danlos, as the structures keep organs in place prolapse.
