Necrotizing Granulomatous Orbital Inflammation Associated with Crohn Disease
A 30-year-old woman with Crohn disease presented with progressive bilateral periocular swelling, ptosis, conjunctival injection and chemosis, severe proptosis, ophthalmoplegia, and periorbital pain (A1–A3). Magnetic resonance imaging demonstrated diffuse infiltration of the lacrimal glands, orbital fat, extraocular muscles, and periorbital soft tissues (B). After orbital biopsy (C), histopathology (D) revealed granulomatous vasculitis with palisading epithelioid histiocytes (arrows), fibrinoid necrosis (asterisk), and necrotizing and nonnecrotizing granulomas in a background of fibrosis (not shown).

Frontal Sinus Bone Wax Complicated by Cyst Masquerading as Meningioma Recurrence
A 70-year-old man presented with left periorbital pain. He had a history of frontal lobe meningioma resection 9 years prior. Examination revealed left proptosis, inferior globe displacement, and restricted upward motility. Computed tomography showed a mixed-density lesion in the superior left orbit, with a partial defect of the superior orbital wall. Magnetic resonance imaging demonstrated a cystic extraconal lesion, isointense on T1 (A), hyperintense on T2 (B), with peripheral enhancement (C) and inferior globe displacement.

Presumed Multifocal Orbital Recurrence of Pleomorphic Adenoma after Orbitotomy
An 80-year-old woman presented with 4-mm relative left-sided proptosis over several years. Approximately 30 years previously, a pleomorphic adenoma of the left lacrimal gland had been excised via lateral orbitotomy with intraoperative frozen section. Computed tomography showed multiple discrete nodules involving the left orbital roof, lateral wall, and temporal fossa (arrows) with bony remodeling and focal defects of the frontal sinus and anterior cranial fossa (A–D). The multifocal distribution was attributed to seeding secondary to violation of the original tumor’s capsule.

It's interesting that most of the things that Wessely mentions that "switch the other way" are just yet more examples of what Davey Smith mentions.

Visceral proptosis [sic] (or visceral ptosis or visceroptosis) is commonly seen in Ehlers-Danlos, as the structures keep organs in place prolapse.
