Horrendous case of acute cellular rejection. Severe tubulointerstitial inflammation and vascular rejection. Plasma cell rich which is often recalcitrant and often seen w/treatment non-adherence (be sure to r/o BK and PTLD as well). #renalpath #pathsky #nephsky

Here is today's #eyeSCANdy. Shown is an acellular SEM findings in segmental membranous GN showing a capillary loop confluent zone (arrow) of GBM crater formations.

Of note, endocarditis-associated glomerulonephritis can also present with a pauci-immune and crescentic morphologic pattern and may warrant exclusion in at-risk patients or if clinical concern exists.

However, it is important to understand that this finding is not entirely specific to infection and can rarely be seen in a range of kidney diseases (such as C3 glomerulonephritis).

Clinicopathologic and Outcome Characteristics of Glomerulopathy With Striated Deposits: A Case Series
Glomerulopathy with striated deposits (GSD) is a rare entity defined by ultrastructural deposits with striated substructure. Prior data were limited to case reports. This case series characterizes GSD...

However, this finding is encountered in fibrillary glomerulopathy in approximately 10% of cases. And, while it may necessitate a paraproteinemia workup, in those with no paraproteinemia it is likely clinically insignificant.

Of note, myoglobin casts and hemoglobin casts cannot always be distinguished from each other on morphology alone. And, in most cases immunoperoxidase staining for hemoglobin and myoglobin can easily facilitate differentiation.

Interestingly, the perihilar focal segmental glomerulosclerosis identified here was present in numerous glomeruli as well as marked glomerulomegaly.

This photomicrograph of a Jones silver stain is interesting in that it shows two findings. First, at 10 o’clock an area of hyalinosis at the perihilar region is present and compatible with perihilar focal segmental glomerulosclerosis.

As suspected, extensive accumulation of intra-podocyte myeloid bodies (a.k.a. zebra bodies, myelinosomes) are seen in all podocyte cell bodies examined (see image below). And, subsequent testing revealed Fabry disease in this patient.

These photomicrographs show a membranous pattern glomerulopathy with C3 staining by immunofluorescence. While it is tempting to diagnosis this as C3 glomerulopathy, that is not correct in this case.
