But I do know that they will not get the same care in the community, or the same access to excellent primary care, mental health, or subspecialists if the VA is gutted

Yes. And by eliminating part time positions, Veterans will loose access to subspecialists. Most smaller VAs can’t support full time subspecialists so they’re paid 2/8 or 3/8ths of their salary and the rest is covered by their university. Now those positions are at risk.

Pediatricians and pediatric subspecialists will be especially hard hit. They are among the lowest paid and subspecializing does not lead to significantly higher pay for pediatricians. So expect more med students to avoid becoming pediatricians and more pediatricians to avoid subspecializing END/

This means less physicians in general, and less physicians in primary care as med students with high interest private loans will be incentivized to specialize in higher paying fields. Additionally, subspecialists will be harder to come by to because of changes to loan forgiveness 3/

Pediatric specialties filled 77.5% positions in the 2024 Match, a decrease of 1.7% compared to last year. AAP thinks it comes down to money - peds subspecialists are paid less for longer training 😢

Yes but a lot of that relates to extreme wait times for specialists and subspecialists. Also access to testing, which can similarly be bottlenecked and subject to excessive waits. I don't see how AI solves the problem of: "there are only 5 subspecialists in the country and 4 of them are in Toronto."

Medicine subspecialists are spending 15-30 additional minutes working in the EHR for every hour spent seeing patients... not surprising per se but easy to understand why full time outpatient practice feels so untenable right now academic.oup.com/jamia/articl...

I have kept mine. Mostly because institutions have different requirements. I don't plan to leave Cincinnati (which doesn't require it for subspecialists), but life takes turns and you never know...

Some of us warned 30+ years ago that the shift to subspecialists and concentrating certain surgeries in tertiary care hospitals would do this.

We need to nurture and insist on physicians who are able to deal with any neurological symptom and also clinics where specialists from
