El seguimiento a largo plazo de un ensayo clínico ha demostrado la efectividad de bloquear la proteína GDF-15 mediante el anticuerpo monoclonal visugromab en entre el 15% y el 20% de pacientes con tumores avanzados que ya no respondían a la inmunoterapia convencional y carecían de otras opciones terapéuticas. Esa es la principal conclusión de un estudio internacional, esponsorizado por la farmacéutica alemana Catalym, liderado por el Cancer Center Clínica Universidad de Navarra (CCUN) y en el que han participado varios hospitales españoles. La investigación comenzó en 2021 y presenta ahora datos a largo plazo, recogidos cuando más del 90% de los participantes había finalizado el tratamiento. Los primeros resultados de esta estrategia pionera de tratamiento se publicaron hace dos años en la revista Nature y ya mostraron el potencial de esta nueva vía de inmunoterapia. GDF-15 es una proteína que aumenta cuando las células tumorales están sometidas a estrés y que aparece frecuentemente en niveles elevados en la mayoría de los subtipos de cáncer. Por ello se estudia como biomarcador, ya que una concentración alta puede estar relacionada con una enfermedad más avanzada y con una menor capacidad del sistema inmunitario para atacar el tumor. El objetivo del estudio ha sido evaluar a largo plazo la eficacia y la seguridad de combinar dos anticuerpos monoclonales, nivolumab y visugromab, en pacientes con tumores sólidos avanzados que habían recaído o progresado durante un tratamiento previo con inmunoterapia. Según el doctor Ignacio Melero, codirector de Inmunología e Inmunoterapia de la Clínica Universidad de Navarra y del Cima Universidad de Navarra, "el estudio ha pretendido comprobar si el bloqueo de GDF-15 podía revertir la resistencia a la inmunoterapia y generar respuestas tumorales profundas y duraderas en diversos tipos de cáncer. Los datos de biomarcadores son prometedores y es posible que pronto podamos identificar a los pacientes que se benefician del tratamiento". Para el estudio se incluyeron 77 pacientes, de los cuales 28 padecían carcinoma hepatocelular, 27 carcinoma urotelial y 22 cáncer de pulmón no microcítico no escamoso. Todos ellos recibieron la combinación de los dos fármacos hasta la progresión de la enfermedad o la aparición de una toxicidad inaceptable. Los investigadores tomaron como variable principal la tasa de respuesta objetiva, es decir, cuántos pacientes lograron una reducción relevante del tumor. "El tratamiento ha demostrado ser muy seguro para los pacientes y ha conseguido que entre el 15% y el 20% de los que eran refractarios a la inmunoterapia convencional obtengan un beneficio clínico relevante, incluso con respuestas radiológicamente completas y duraderas", ha destacado el doctor Melero. Por su parte, la doctora María Rodríguez Ruiz, especialista en Oncología Radioterápica de la Clínica, ha añadido que "esta línea de investigación tiene gran potencial para mejorar notoriamente el tratamiento de enfermedades oncológicas muy prevalentes".
