ACLF East vs West: APASL—acute insult in CLD → jaundice/INR, liver failure first; HBV common; focus on liver rescue (antivirals, PE, albumin). AASLD/CLIF—decomp cirrhosis w/ multiorgan failure; sepsis/alcohol common; ICU + early LT. Synthesis needed.
