It's weird that everyone and their carelessly parented child is getting GLP-1s right now but if I, a type 2 diabetic, want to get on a GLP-1, I have to go through a lengthy escalation ladder through the biguanides and various other drugs before my doctor will even consider it

Every time I start a new diabetes medication, or a new dose of my diabetes medication, it gives me clarity and energy for a while ... and then my body seems to adjust. Annoying and distressing.

I've asked my doctor if we can do a GLP-1. He won't do it unless we try biguanides first.

Researchers from Zhejiang University and UC Davis studied how metformin works differently from other biguanides.

Circling back to the GLP-1s, I have type 2 diabetes and so you’d think I would qualify. My doctor has indicated he’s unwilling to prescribe unless my blood sugar is not adequately controlled on biguanides (metformin, etc). Unfortunately, my blood sugar is just about adequately controlled.

mitochondrial biguanides because they lack metabolic flexibility. LKB1 mutations are highly associated with the development of "cold," immunosuppressive tumor microenvironments that resist traditional immunotherapy.

We show that hydrophobic biguanides also inhibit complex I in a state dependent manner but their inhibition can be explained using a competitive framework. Whereas, due to the trapping mechanism induced by quinone there is positive cooperativity between the binding of metformin and quinone. 3/n
