Because vasopressin is hard to measure (it breaks down quickly), the team also checked copeptin in 13 patients.
Copeptin is a more stable molecule released alongside vasopressin.
Those levels were normal-range but mostly low-normal, reinforcing the low vasopressin finding.

No. Copeptin measures ADH activity. The way we do that in hyponatremia is via urine osm. Unless you are struggling with differentiating tea and toast syndrome (⬇️ ADH) from SIADH (⬆️ ADH) this will not help. The trouble in hyponatremia dx is separating CHF from SIADH which are both ⬆️ADH.

No. Copeptin measures ADH activity. The way we do that in hyponatremia is via urine osm. Unless you are struggling with differentiating tea and toast syndrome (⬇️ ADH) from SIADH (⬆️ ADH) this will not help. The trouble in hyponatremia dx is separating CHF from SIADH which are both ⬆️ADH.

I rarely use copeptin (mostly studied in DI I believe?). Interestingly it helped me diagnose type C SIAD in 2 patients however
