Require transparency down to the contract level from all providers. Maybe to the point of general ledger entries. Segregate formularies from PBMs. End steering. This allows everyone to dig in and see what care would actually cost and determine a path, that everyone can discuss

What's the solution ? Lots of moving parts. PBMs should not do formularies (their leverage ) , no provider networks, publish everything, require producers to publish and honor cash pay, with government guarantees for care individuals can't afford that is means tested.

The whole concept that big PBMs control formularies is INSANE. In a world of biosimilars and generics, they will prevent a Humira biosimilar like Yusimry, which is $594 per month from being available so they can offer a biosimilar that pays them a huge rebate. Costing yr company/premiums more

The Big PBMs have this leverage because they use the formularies to intimidate manufacturers from selling to other Group Purchasing Orgs. This is exactly why brand meds are so expensive. I don't know the exact %, but almost all brand sales get rebates through GPO contracts controlled by big PBMs

If the bill passes, big PBMs will find new fees to charge manufacturers, sponsors and pharmacies. They will use formularies as leverage.

The big PBMs say to manufacturers , "we cover 25m lives. If you want your drugs to be offered to those 25m people , you have to not only give us rebates

the cost of medications alone can be ridiculously exorbitant -- then we look back at the insurance companies that deny claims or medication coverage for this & try to axe medications needed for survival from their formularies. preventative care & health maintenance can be unaffordable.

From my research lots of formularies have stopped accepting sleep apnea as a valid criteria for new coverage. Even with my sleep apnea and past T2D diagnosis, nothing short of an A1C over 6.5 will get me GLP-1s again.

Pharmacy benefit managers (PBMs) performed a useful function when they were formed last century. They managed formularies and payments, and they pushed back on drugmakers. But, then PBMs got greedy.

ryan white is considered a payor of last resort in most or all cases as well. if ryan white is having to pick up the costs of the medications, then that's also telling us that several insurers don't have current HIV medications in their coverage formularies.

Lol. Except for brands , they don't negotiate prices. They sell access to formularies in the form of rebates

Find me one PBM that publishes a price list. Anyone can get our price list
