I don't think any particular *dungeon* itself stood out as "this dungeon is awful for Kara," but there are definitely two Endwalker dungeons notable for the horrible mental state Kara was in at the time that she did them, even if she was little-fazed by the dungeons themselves.

Kara and Kazimiera both have no piercings. Ouch! I wouldn't want someone drilling a hole through my HORN.
Whenever they want/need to wear "ear" ornamentation, they use clips, hooks, wraps, etc. instead. Kara frequently does this with the Wayfarer's Earcuff.

Kara generally tries not to let others see her cry—she does it in her room, alone, quietly but fiercely.
Her crying is almost always caused by frustration at her own self-perceived failures or shortcomings—to understand, to protect, to fit in or find a sense of belonging, etc.

I've said before and I'll probably say it again, but I consider Kara "gender-agnostic demisexual."
She doesn't heavily buy in to gender or sexuality as identities in the first place. People are people. She's not even sure why gender-distinct pronouns exist in the first place.

Kara is extremely uncomfortable with it for a long time, and basically hides under large robes and hats until the start of 3.2. Even after that, she still primarily wears low-exposure outfits that are more "coat or tunic" oriented, such as her iconic Bozjan Coat.
