Yes you’re probably referring to Uromune (MV140), a sublingual bacterial immunotherapy for recurrent UTIs. It contains inactivated E. coli, Klebsiella pneumoniae, Proteus vulgaris, and Enterococcus faecalis. The evidence is interesting, although I think social media gets ahead of where the data are

Composition of MV140 MV140 is a sublingual vaccine, a type of vaccine which doesn’t use needles, but enters the body through tablets taken orally, or through a spray. They’re made of inactivated bacteria, and the notion is that if you put (a tablet-based sublingual vaccine) under your tongue and it was daily for three months, your body would recognize these dead bacteria, mount an immune response at the level of where it should so that when you had an infection, you would mount your own immune response, not needing an antibiotic. In this case, MV140 just happens to taste like pineapples!🍍 Proposed mechanism of action for MV140: An episodic UTI from a uropathogen entering the urinary tract causes an acute infection with inflammation that typically resolves with antibiotic therapy. In some cases, in susceptible individuals, the epithelial damage results in an increase in T helper 2 (Th2) response, which impairs resistance to both exogenous bacteria (from outside the urinary tract) and endogenous bacteria (within the patient’s urinary microbiome) causing a scenario of recurrent UTI. MV140 increases Th1 and Th17 cells which both downregulate the abnormal Th2 response and promote or mediate cellular immune responses, resulting in a decreased risk of rUTI from bacteria outside and inside the bladder microbiome. Here I am! We don't have access to mv140 but have Urovaxom (similar), a lysate of killed E coli. I'm the wrong person to ask re effectiveness bc anyone who comes to our clinic has already tried & failed everything.. Clinical data are mostly from the 80s (methodologically flawed, not super helpful).

Wie genau die Immunabwehr durch MV140 angestoßen wird, sei noch nicht abschließend geklärt. Aber klar sei, dass er beim Sprühen unter die Zunge direkt an der dortigen Schleimhaut wirkt. „Und die steht wiederum in Kontakt zu den Schleimhäuten in den Harnwegen, sodass es dort zu einer besseren Immunantwort auf Infekte kommt"® ", so der Urologe.
