In a retrospective study, urinalysis detected 92.8% of single-uropathogen bacteriuria. Most positive cultures after normal urinalysis represented asymptomatic bacteriuria. An exploratory symptom- and urinalysis-guided model projected 68.9% lower direct laboratory costs.

UTI and bacteriuria during pregnancy: one and the same?

Antibiotic stewardship draws a clear distinction between symptomatic UTI and asymptomatic bacteriuria (ASB). But does pregnancy change the picture?

🔥 Brilliants minds debate oral fosfomycin for complicated UTI including pyelonephritis. With increasing ESBLs, we need options for UTI beyond cystitis. Oral fosfomycin is an appealing option, but is it ok for infection outside the bladder?

“Development of the ExPEC9V vaccine has been discontinued.”

So disappointing - but good to have robust evidence. 😢

Whats in the papers?

E.mbrace Trial, Bonten et al.:

In this large phase 3 trial of older adults, ExPEC9V did not demonstrate efficacy in preventing first IED caused by ExPEC9V O-serotypes, despite generating robust and sustained antibody responses.

We examined the phenotypic characteristics of K. pneumoniae UTI isolates cultured in human urine vs LB. Despite diverse behaviors, these strains consistently cause pyelonephritis in a mouse UTI model.
