Our molnupiravir work is now out after peer-review! We definitively demonstrate that molnupiravir has resulted in viable SARS-CoV-2 viruses with significant numbers of mutations, in some cases with onwards transmission of mutated viruses. www.nature.com/articles/s41...

1/ PolyBio’s Dr. Sara Cherry asserts: “Treating [the lung] with either nirmatrelvir (Paxlovid) or molnupiravir, we clear the [SARS-CoV-2] infection by 10,000-fold… while nirmatrelvir clears the infection in intestinal cultures, molnupiravir has no activity.”

Study of >74K older Covid patients: When given early, the antiviral drug molnupiravir (Lagevrio) *halved* the risk of death.

Molnupiravir was effective regardless of sex, DCCI score [which measures comorbidities], age in patients over 65 years, hospitalization status, & Covid vaccination status.

The evidence shows that molnupiravir is not an effective drug & is likely even harmful for immunocompromised patients. 8/11

By far the largest & best randomized trial of MOV—& the only one not run by Merck—was PANORAMIC, run by UKHSA. It found no benefit from MOV & hinted MOV treatment was harmful for the immunocompromised, whose hospitalization/death rate was double in the MOV-treatment compared to non-MOV group. 6/7

BA.3.2 emerged in Nov 2024 after ~3 years of intrahost evolution with >50 new spike AA muts, but since then, it's changed very little.

Could the mutagenic drug molnupiravir (MOV) galvanize BA.3.2 into pursuing new evolutionary paths? A new 89-mut BA.3.2 sequence hints that it could. 1/11

Not quantified or elaborated on.

"Exceeded" seems an understatement: looks like ~7X more, from the ADHAC data.

Study ignores the elephant in the room - Molnupiravir has been withdrawn in most countries.

I've previously noted that recent molnupiravir sequences (meaning sequences whose mutations clearly originate in MOV treatment) have shown a marked trend toward positive selection of MOV mutations, i.e. MOV muts are increasing viral fitness. 1/7

Particularly interesting to me is the recent trend for *positively selected* MOV-induced mutations. Non-synonymous transition mutations score FAR higher for MOV nucleotide contexts than synonymous mutations, indicating these MOV mutations are very likely advantageous. 3/4

Australia, Taiwan, and Japan seem to be the major molnupiravir users these days. Sometimes it's hard to tell if a new MOV sequence is from recent MOV use or treatment from the past—like this recent FL.20.1 whose closest relatives on the tree were collected ~1 year ago. 1/4

It looks like in Australian Aged Care settings, they are still dispensing Molnupiravir at around 4X the rate of Paxlovid.

I thought I heard Paxlovid was preferred now? Doesnt seem to have filtered through to the outcomes.

NICE final draft guidance for molnupiravir published: www.nice.org.uk/guidance/ind... . In a change from the draft from late 2024, and the original draft decision from 2022, it now recommends molnupiravir for subgroups at risk of severe COVID-19 for whom other treatments are unsuitable. 👀

Another molnupiravir sequence from Australia showed up today w/extremely strong evidence of positive selection for MOV-induced mutations, a sign the genetic diversity created by MOV is being utilized to the virus's advantage.

Similar signals have been in almost every MOV sequence in recent months.

Sequences resulting from failed molnupiravir treatment still show up now & then & still almost invariably display strong signs of positive selection of MOV-induced mutations (suggesting MOV muts are increasing viral fitness). Mostly from from Japan & Australia now. Latest is an NB.1.8.1 from Aug 4 👇

Another day, another sequence displaying undeniable evidence of positive selection of molnupiravir-induced mutations (i.e. adaptive mutations—good for the virus, bad for the infected person).

G->A is the classic MOV mutation—and the vast majority of the positive selection is in G->A muts. 1/3
