Many of these supposedly avoidable admissions were among elderly frail patients. The headline diagnosis was often urinary tract infection - which is preventable. But dig into secondary diagnoses you often see considerable multimorbidity, and the UTI diagnosis becomes questionable.

Among 3028 PWH in Africa, 36.6% had multimorbidity; 721 hospitalizations occurred (4.06/100 person-yrs). Continued multimorbidity raised hospitalization risk by 27% (aHR=1.27).🦠🏥
Multimorbidity Trajectories and Hospitalization Risk Among People With HIV in Sub-Saharan Africa: A Longitudinal Analysis From the African Cohort Study
Increasing multimorbidity among people with human immunodeficiency virus (PWH; HIV) in sub-Saharan Africa can result in hospitalization. We investigated longitudinal associations between multimorbidity trajectories and hospitalization risk within the African Cohort Study (AFRICOS).MethodsAFRICOS enrolls people with and without HIV from HIV clinics in Uganda, Kenya, Tanzania, and Nigeria. These analyses restricted PWH aged ≥18 years enrolled between 2013 and 2024. Multimorbidity (≥chronic conditions in addition to HIV) was ascertained from International Classification of Diseases codes and clinical measurements. Multimorbidity was categorized into 3 trajectories: resilient, developed, and continued. Hospitalization reasons were ascertained by medical record review. Andersen–Gill extensions of Cox proportional hazards models estimated adjusted hazard ratios (aHRs) and 95% confidence intervals (CIs) for the association of multimorbidity trajectory and hospitalization.ResultsAmong 3028 PWH, 36.6% (n = 1109) had multimorbidity at enrollment. Over 17 740 person-years, 721 hospitalizations occurred (incidence density: 4.06 per 100 person-years). Systemic infections (n = 209, 29.0%), predominantly malaria (n = 182, 87.1%), led admissions. In adjusted models, PWH with continued multimorbidity had a significantly higher risk of hospitalization (aHR = 1.27, 95% CI: 1.06–1.53) compared with resilient PWH.ConclusionsPWH with continued multimorbidity have a significantly increased risk of hospitalization. Intervening early with optimal ART and comprehensive chronic disease management is critical for improving long-term outcomes in sub-Saharan Africa.

Harmonization of transdiagnostic psychiatric phenotypes across a large multi-site study reveals high burden of neuropsychiatric multimorbidity in 16p11.2 and 22q11.2 copy number variant carriers
CONCLUSIONS: The C2PS is a pragmatic and reliable tool to harmonize heterogeneous neuropsychiatric phenotypic data for large multi-national studies. Observations in 1,325 individuals with CNVs at 16p11.2 or 22q11.2 reveal variant-specific psychiatric profiles, emphasize burden and impact of psychiatric multimorbidity, and provide insights beyond categorical diagnoses.

JMIR Public Health: Temporal Patterns of Multimorbidity Progression Among Inpatients With a Recorded Hypertension Diagnosis Across Three Counties in Rural Henan, China: Retrospective Cohort Study Using Temporal Network Analysis
Temporal Patterns of Multimorbidity Progression Among Inpatients With a Recorded Hypertension Diagnosis Across Three Counties in Rural Henan, China: Retrospective Cohort Study Using Temporal Network Analysis
Background: Hypertension is a key driver of multimorbidity; however, its role in shaping dynamic disease progression, especially in rural populations, remains underexplored. Objective: This study aimed to characterize temporal patterns of multimorbidity progression among inpatients with a recorded hypertension diagnosis and to examine whether these patterns differed by sex, age, insurance type, and county. Methods: We analyzed electronic health record data from 34,099 inpatients with a recorded hypertension diagnosis across 3 rural counties in Henan, China (2019‐2023), constructing a temporal comorbidity network of 250 disease nodes and 1512 disease transitions. Statistical validation was performed using bootstrap resampling, case-dropping, and permutation tests. Subgroup analyses were conducted by sex, age, insurance type, and county. Results: The temporal comorbidity network revealed a densely interconnected core involving hypertension, atherosclerotic heart disease, and cerebral infarction, with type 2 diabetes occupying a bridging position between cardiovascular and multisystem disease clusters. The ordering of dominant recorded pathways differed by sex (earlier coronary involvement among female inpatients vs more direct progression to stroke among male inpatients), was more complex in inpatients older than 70 years of age, and was more diffuse among those covered by urban-rural resident insurance compared with urban employee insurance. Conclusions: Multimorbidity among inpatients with recorded hypertension followed structured and heterogeneous temporal patterns that differed by sex, age, insurance type, and county. Cardiovascular and cerebrovascular conditions formed the principal recorded progression pathways, while type 2 diabetes connected cardiovascular pathways with broader multisystem complications. These findings support further evaluation of risk-stratified and locally adapted approaches to chronic disease #Prevention and longitudinal care.
