This figure shows the design of the study herein that analyzed peripheral blood mononuclear cells (PBMC) in patients with Post-Acute Sequelae of COVID-19 (PASC). The results revealed significant differences in PASC compared to persons with no history of COVID-19 (No COVID) and those with a prior history COVID-19, but with full recovery (COVID + ). The study elucidates that PASC patients exhibit altered mitochondrial function, increased ATP-linked respiration, and increased oxygen.

This study documents long‑term differences in PBMC transcriptional states in people with LC symptoms.
However, it is cross‑sectional, PBMC‑restricted, and associative. Claims about prediction, causality, and disease mechanisms go well beyond what the data can support.

Tofacitinib influence on cellular interactions at the synovium and vascular sites and the effects on endothelial dysfunction during inflammation

Tofacitinib treatment leads to reduction of inflammation at the synovial and vascular levels. In cocultures between activated PBMC and synoviocytes, tofacitinib massively inhibits the production of IFNγ and to a lesser degree IL-17A and the anti-inflammatory cytokine IL-10. In cocultures between activated PBMC and endothelial cells, tofacitinib results in great suppression of IFNγ, IL-10, IL-6, and IL-8 production and partial inhibition of IL-17A. Tofacitinib reduces IL-6 production by ECs activated after the addition of pro-inflammatory cytokines (eg, TNF + IL-17A) but does not modulate the pro-adhesive and thrombotic potential of those cytokines on endothelial cells, notably the increase in tissue factor and adhesion molecules expression and the downregulation of thrombomodulin.
