NPR really both sidesing one of the best medical breakthroughs of the past 5 years.

We have had RSV monoclonal antibodies for 2 decades (palivizumab). It was expensive, tricky to dose, and had to be given multiple times per season. The new ones are great.

I had plenty of criticism for Cody Meissner’s positions related to pediatric Covid vaccines, but he’s a Peds ID Doc with deep clinical trials experience (instrumental work on RSV prevention with palivizumab - synagis™️).
He called out the bullshit pretense of today’s ACIP meeting. I hope he resigns.

RSV infects 90% of children by age 2 and causes over 3 million hospitalizations annually. Palivizumab & nirsevimab are injectable monoclonal antibodies used for prevention in infants. #IDsky #MEDsky #PEDSsky

The previous generation monoclonal antibody, palivizumab, had strictly defined high risk indications such as prematurity and congenital heart disease. This was due to the high cost and short duration of protection.

We also had a functional ACIP at that time.

I concur. As a practicing pediatric pulmonologist for almost 30 years I am amazed how effective palivizumab and now nirsevimab have been in reducing the hospitalization rates due to RSV in babies. Nirsevimab is given to all babies and works. I’ve not seen sig side effects from either medication

There is another drug called Synagis that has been available longer than Beyfortus, but the requirements to qualify are more strict. www.synagis.com/index.html

Mas há mais na ignorância. Há anos que se utiliza o Palivizumab contra o vsr em grupos de risco. Porquê a troca? Porque o Palivizumab precisa de 5 doses durante a época epidemica, o novo precisa de 1 dose.
O nirsevimab é caro por dose, mas mais barato por criança.

8/ Treatment for RSV: 🌡️ Most cases are treated with supportive care:
Hydration 💧.
Nasal suction for congestion 🫁.
Oxygen therapy if severe. 💉
For high-risk infants, Palivizumab (antibody therapy) may prevent severe illness.
🚫 Antibiotics won’t work (it’s viral).

Nice paper looking at regional changes in RSV hospitalisation in Nova Scotia, Canada, when palivizumab eligibility stopped for 30-32 week babies. From 0 hospitalisation to 9% in this group.

Während gegen SARS-CoV-2 & Influenzaviren wirksame Impfstoffe relativ breit eingesetzt werden, waren die Möglichkeiten zur Prävention schwerer RSV-Infektionen bis dato beschränkt auf den seit über 20 Jahren verfügbaren monoklonalen Antikörper Palivizumab für Risikokinder unter 2 Jahren.

There were also differences in risk for persistent symptoms too:
RSV = hypoxemia, hospitalisation, & prematurity. Palivizumab was protective

SARS-CoV-2 = hospitalisation, & elevated inflammatory markers

Influenza = prolonged fever, acute-phase fatigue/myalgia, & hospitalisation

Ten eerste zijn er meedrere producten op de markt; Nirsevimab (AZ/Sanofi), Palivizumab (Synagis) en Clesrovimab (Merck). Het klopt dat dit monoclonale antilichamen zijn. Ideal, en dat zal in de toekomst misschien ook gebeuren, worden er meer dan 1 tegelijk gegeven.

Sooooo

Synagis (palivizumab) has been around for 2 decades but it is significantly more expensive and has to be re-dosed every month. It is only given to the most at risk patients.

So the idea isn't exactly new but the ability to protect all babies is a potential game changer.

If it's available and the parents consent buuuuuut...

Nirsevimab and Palivizumab are not immunizations in the classic sense in that they do not illicit an immune response. They are monoclonal antibodies.

Clesrovimab has been modified with the YTE mutations to enhance its half-life to 44 days. Clesrovimab has been assessed in a Phase 2b/3 study compared with placebo (IMO it should have been compared with palivizumab).
