Our local enthusiasm for phenylephrine could use a gentle re-think.

Still annoyed that they† spent 20 years trying to sell us phenylephrine, even though we had studies going back to the 1970s showing that it was no better than a placebo.

† I have no idea who "they" actually is.
How Two Pharmacists Figured Out That Decongestants Don’t Work
A loophole in FDA processes means older drugs such as those in oral decongestants weren’t properly tested. Here’s how we learned the most popular one doesn’t work

Ours too. Once OB started using phenylephrine for everything we've seen its use expand into other parts of our practice. We generally fall into the pattern of "give multiple anesthetics at generous doses, pick up the pressure with phenylephrine."
It makes my minimalist heart sad.

try to avoid mixes with phenylephrine or decongestants, if you want to help clear mucus I'd recommend looking for the mix with guaifenesin and dextromethorphan only which helps clump the mucus and suppress cough

Think the issue with peripheral norad, in theatre at least, is I can't just directly replace the metaraminol/phenylephrine, especially as I'm pretty aggressive in my vasopressor usage.

bsky.app/profile/rajl...
Peripheral norad is still quite new here so I think it'll take a while before people are comfy with it. Even @assocanaes.bsky.social guidelines are fairly prescriptive about it. Only ever bolused it in extremis. Good to know the dose though!

That's is quite unhelpful.

Just used to put the phenylephrine 10mg/ml in a 100ml bag. Useful for section lists and emergency theatres where you use it a lot.

Not done sections for years though.

On reflection (there'll be a lot of this, sorry), I think I was a bit miserable at the start of the trip because I had a stinking cold and cough.

Not ideal to be lugging the rucksack around with a congested head and lungs in countries where you can only get phenylephrine on prescription.

I've seen dilute metaraminol in ampoules (0.5mg/ml) and concentrated phenylephrine (10mg/ml) so think this is centre-specific. Difficulty with noradrenaline is you can't give it as bolus which a lot of my colleagues use it for.

Oh, right, I forgot about them removing the phenylephrine.
October 25, 2026 at 12:02 PM
fine, get rid of the phenylephrine…but you will have to pry the guaifenesin out of my cold dead snot
October 25, 2026 at 11:20 PM
In the US its the opposite (in a bad way)... you get cold medicine but instead of just phenylephrine (the decongestant) u get acetaminophen & a bunch of other bullshit so they can jack up the price... hundreds if not thousands of ppl go to the hospital for liver toxicity every year cuz of this
October 24, 2026 at 3:30 PM
Just remember, kids: phenylephrine is a placebo, so if you can't take pseudoephedrine (high blood pressure, etc.), go for alternatives like nasal sprays :)
October 24, 2026 at 4:28 PM
“Americans collectively shell out $1.763 billion a year for cold and allergy meds with phenylephrine, according to the FDA, which also calls the number a likely underestimate. That’s a lot of money for a decongestant that, again, does not work.”

In Australia they pulled ephedrine and pseudoephedrine from shelves on the basis that the risk outweighed the benefit when phenylephrine was available. So frustrating!

Cold/flu season can complicate a life with #epilepsy. Common OTC decongestants like pseudoephedrine or phenylephrine can lower seizure thresholds or interact with anti-seizure medications. -- Do you have more seizures during this season? #EpilepsyAwareness

Good journalism. I’ve included a gift article link.

Main point: phenylephrine does *not* work when ingested (for example, in Sudafed PE). It does work as a nasal spray.

Also: pseudoephedrine *does* still work. You have to ask the pharmacist for it, but it’s worth it.
