This is a puzzling conclusion. The authors assess how self versus nonself antigen reactivity influences tTreg stability in the periphery. However, no self antigen is specified or tested. Considerable effort, yet little support.

(1) Thymic rejuvenation: Using agents like KGF, RANKL, or rapamycin to promote thymic epithelial cell proliferation, restoring thymic function and naïve T cell output. (2) Antigen-specific tTreg therapy: Engineered tTregs generated via TCR/CAR engineering or extracellular vesicle (EV) platforms precisely home to digestive tissues to locally suppress autoreactive T cells. (3) Clinical thymectomy: Serving as a natural clinical model, it can cure thymoma-associated paraneoplastic digestive syndromes but impairs tTreg generation and increases long-term autoimmunity risk. Restoring the balance of this axis ultimately aims to re-establish immune tolerance and achieve disease remission.

In vitro, these lines could be restimulated and propagated. The 3 lines, from one subject, all showed stable Foxp3 expression, indicating high stability. This suggests these are thymic-derived tTreg cells. Their number did not expand upon additional vaccinations.
