On a pilot forum someone pulled NASA ASARS incident reports from aircrews for DCA and Helicopter and boy it's disturbing how many airline pilots with similar sounding reports and even the phrase "accident waiting to happen" used by many. cc: @jfallows.bsky.social

Ian was one of four PSA crew members who died in the tragic midair collision last night—and the only one I knew personally. We flew a memorable four-day trip together when I was at PSA. He made every flight fun for our passengers and radiated positivity. My condolences to his loved ones. 💔
