• Gum disease doesn't just stay in your mouth. It triggers body-wide inflammation through adipokines, causes neutrophil dysfunction, and creates advanced glycation end-products that worsen insulin resistance.

Agree! A big question I have been wondering for a long time is how general fat metabolism and dynamics affect the release of adipokines and other lipid mediators of inflammation.

Blood-based obesity biomarkers and their relevance for disease risk | Nature Reviews Endocrinology
Obesity is a risk factor for chronic diseases and early death; however, the underlying mechanisms are not fully understood. Whereas insulin resistance and inflammation are established pathways in several of these relationships, it is less clear how increases in body adipose tissue relate to these pathways and disease risk. Several adipose tissue-derived blood-based biomarkers have been identified as purported mediators, including adipokines, inflammatory cytokines and sex steroid hormones. Traditionally, these markers were discovered in animal models and their relevance in humans has then been investigated in epidemiological studies. Today, proteomics and metabolomics approaches in human observational studies are used to discover obesity biomarkers in blood, supported by Mendelian randomization studies to draw causal inferences. Here we review adipose tissue-derived blood-based obesity biomarkers and their relevance for disease risk, along with their potential role as mediators. Proteo

Dr. Milton Packer proposes that hypertrophied, inflamed fat cells secrete molecules—adipokines—that drive cardiac remodeling, inflammation, and fibrosis.

In other words: HFpEF may begin in fat, not the heart.

Themes emerge:

• It’s about fat distribution, not just BMI.

• Adipokines act through endocrine-paracrine signaling.

• Sex, race, and metabolic context all shape expression and risk.

• New therapies (GLP-1RA, SGLT2i) may restore adipokine balance.

Visceral fat, which is found in and around abdominal organs including the stomach, liver and intestines, is the driver of metabolic disease, says Kessler. Visceral fat leeches fatty acids called adipokines or chemokines, which cause systemic inflammation in the body.
