Got confirmation for getting back on a bisphosphonate for my bones today (after like a decade of increased pain & crunching bones, AND new Wheelie art? Today's SICK.

Since my last bone scan 6 months ago, I've had reduced metabolic activity in two areas of bone metastases. I'm on a CDK 4/6 inhibitor, aromatase inhibitor, & bisphosphonate line of treatment

I thought "stable" was the best. But this is actual improvement

Sagen wir mal so: ich habe auch oft erlebt, dass Patient*innen wegen sowas Zähne verloren haben oder der Kiefer gebrochen ist.
Stichwort BRONJ.
Rechtfertigt keinen Rant über Bisphosphonate, aber es würde helfen, wenn man sie vor Therapiebeginn zum Zahnarzt schickt und auch regelmäßig kontrolliert.

🦴🔬 A 'forever unpublished' image! This histological section of the human jaw bone shows large amounts of new bone (intensely stained areas) after switching from a bisphosphonate (alendronate) to denosumab as the anti-osteoporotic drug.

Been to the dentist, and I’m still *very* numb so watching the cricket while it all recovers 😱.

I shall be back there again soon for a ‘proactive crown’ prior to resuming bisphosphonate treatment. Terrifying to me, but I love my new dentist, which really helps.

What exasperated me was that in 2023, the oncologists wanted to prescribe bisphosphonate infusion therapy for osteoporosis, which I did not have and still do not have. I have mild osteopenia, which is common in women my age.

What’s the eGFR? 31 is probably different than 59 for the risk of adverse events. I don’t think I would have any concerns re: bisphosphonate but would monitor PTH more closely than usual and if it drops or there is an AE consider an alternative.

There are no good trials I’m aware showing impact on fracture risk in CKD with concomitant low BMD and secondary hyperparathyroidism which presumably is what the patient has

As the benefit is uncertain give the least risky med (if anything!) which is probably the bisphosphonate

The ROS article said patients may be offered bisphosphonate treatment afterwards, so that fits with what you were told. Good luck with it all. And huge respect for injecting yourself! Not sure I’d be so good at that.
