No one quite knows why they show nuclear WT1, but it is very helpful. CADM3 expression is more specific but no one has the antibody. They often contain scattered keratin, or less often Desmin-positive cells. The Brigham group wrote a nice big series. pubmed.ncbi.nlm.nih.gov/35713642/

By IHC it is diffusely CD34-pos, shows strong nuclear WT1, and contains scattered desmin-pos cells. This is a superficial CD34-positive fibroblastic tumor. Very low grade tumor, often with PRDM10 fusions. Jody Carter, now in Edmonton, wrote this paper. pubmed.ncbi.nlm.nih.gov/23887307/

Maybe looking at it more would help? It’s got branching, thick-walled vessels and haphazardly-arranged fibroblastic cells, like an SFT. CD34 is negative, but STAT6 is diffusely positive. This is a lipomatous SFT (previously lipomatous hemangiopericytoma). Not all SFT are CD34+.

This of course is what we now call “myxoinflammatory fibroblastic sarcoma”. Originally reported as “Inflammatory myxohyaline tumor of distal extremities with virocyte or Reed-Sternberg-like cells”. I think this older name helps us remember the distinctive diagnostic features of this sarcoma.
