The combination of hematuria, proteinuria, and casts on a UA is considered “nephritic sediment”. The presence of the sediment helps to localize the pathology to the glomerulus.

Consensus:
The indications can guide you for the biopsy:
🔬Age <30 or >60
🔬Active urine sediment
🔬AKI/Acute Nephritic presentation/RPRF
🔬Atypical symptoms: arthritis / rash
🔬Better to avoid in DM pts with CKD & proteinuria without atypical features

Signs of GPA

The combination of an acute kidney injury with a nephritic sediment and any upper respiratory symptoms (sinusitis, otitis, nasal deformity) should immediately raise suspicion for GPA.

Management starts with a comprehensive complement workup: C3, C3c, Bb, C4, nephritic factors, soluble C5b-9, properdin, factor H, CH50/APH50, and genotyping. Treatment is activity-guided: supportive care → MMF/steroids → anti-complement therapy.

Acquired factors matter equally: C3 nephritic factors (present in 50–80% of patients) stabilize C3 convertase and drive uncontrolled activation. Highly stabilizing activity correlates with rapid disease progression.

A nephritic puzzle: C3-dominant glomerulonephritis as a sentinel of hidden autoinflammatory disease - Pediatric Nephrology
Membranoproliferative glomerulonephritis (MPGN) is among the most challenging glomerulonephritides to diagnose and manage, given its clinico-immunopathologic heterogeneity. Although recent advances…
